How to book an appointment

  • 2. Go to "Cudzoziemcy" section
    • Navigate to Cudzoziemcy and select your application type.
  • 3. Check slots
      • Pick the earliest available slot. Book at least 3-4 weeks ahead.
  • 4.Print the confirmation
      • Print the confirmation and bring it to the appointment.

Documents

Base package: always required

  • Application form (wniosek) from duw.pl. Filled in Polish.
  • 4 photos 35x45 mm, white background, no glasses, under 6 months old.
  • Passport: original plus copies of all stamped pages.
  • Rental agreement or owner's statement (oświadczenie właściciela).
  • Fee of 340 PLN for a temporary permit (640 PLN for permanent), proof of payment attached. The 100 PLN card fee is paid later, at pickup.
  • Health insurance for the full permit period, or proof your employer pays ZUS.

Based on work or incubator

  • Employment or B2B contract, signed, in Polish or with a sworn translation.
  • Income confirmation from employer or incubator.
  • If via an incubator: contract plus invoices and payments from the last 3 months.
  • ZUS no-arrears certificate via e-ZUS.

Lower Silesia specifics:

The office only accepts documents on the booked date. No walk-ins. Slots open at 8:00 AM on duw.pl and go fast on popular days. Book 3-4 weeks out.
